Information
If your child was born at home in
Houghton County without a midwife, you need to file your
home birth at the Houghton County Clerk’s Office. Please
come with your completed paperwork no later than 3:00
p.m. The paperwork is available upon request at the
Clerk’s Office.
If a birth occurs outside an
institution, a Michigan Live Birth Worksheet must be
completed and filed with the County Clerk within one (1)
year of the date of birth.
The birth must have
occurred within Houghton County to file the record at
the Houghton County Clerk’s Office. The following must
be present for the County Clerk’s Office to process the
filing:

At least one (1) parent, with their valid state-issued ID or valid driver’s license.

If both parents are present, they would both need their IDs to sign the birth certificate.

The child (within 1 year of birth).

Michigan Live Birth Worksheet, completed and signed by the informant and the Certifier.

Proof of pregnancy and proof of live birth must be provided.

You may request certified copies of the birth record for a fee, once it has been placed on file.
NOTE: If a midwife assisted with
delivering the child, they have likely submitted the
necessary documents with the State to file a Birth
Certificate. If a midwife was involved, parents can
request a Certified copy without having to register
anything additional with the Houghton County Clerk’s
Office.
